JOB DESCRIPTION
We are seeking a dedicated and adaptable PRN Registered Nurse (RN) to join our hospice team. The ideal candidate will possess a strong clinical background, excellent assessment skills, and a compassionate approach to patient care. As a PRN RN, you will provide flexible, high-quality nursing services in patients' homes, supporting symptom management, medication administration, and overall comfort. RESPONSIBILITIES
- Conduct comprehensive patient assessments to evaluate clinical needs and develop individualized care plans.
- Administer medications, including IV infusions and injections, following aseptic techniques and safety protocols.
- Manage complex symptom control such as pain management, dyspnea, and other palliative concerns.
- Collaborate with interdisciplinary teams to coordinate care, including mental health support and behavioral health interventions.
- Document patient interactions accurately using electronic health record (EHR) systems such as Epic or Cerner, ensuring compliance with HIPAA regulations.
- Provide education to patients and families about disease processes, medication administration, and advanced care planning.
- Respond promptly to urgent patient needs through triage protocols and critical care procedures when necessary.
- Support hospice and palliative medicine initiatives by maintaining high standards of infection control and safety practices.
QUALIFICATION
- Proven nursing experience in Hospice
- Prior involvement in home care or community-based nursing enhances the ability to deliver patient-centered services effectively.
- Critical care experience including ventilator management, ostomy care, or trauma medicine is beneficial.
