White House Healthcare Center, a privately owned skilled nursing facility in Orange NJ is seeking a Part Time Social Worker to join their team of highly skilled professionals. Social Worker responsibilities include but are not limited to:
- Develop preliminary/comprehensive assessments for each resident.
- Report any issues/ changes to the Director of Social Services.
- Encourage the resident/family to participate in the developmental process and review of resident’s plan of care.
- Develop a written plan of care for every resident that identifies all social problems and needs of the resident, followed by setting goals to be accomplished for each problem/need identified.
- Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner.
- Review nurse’s documentation to determine if the care plan in place is being followed properly.
Job Type:
- 16-24 hours a week between the hours of 9-5
License/Certification:
- 1 year of nursing home social worker experience required
- CSW or LSW required
