Minimum Requirements
Candidates must have at least 2 years of home-based healthcare experience within the last 10 years, including one or more of the following:
Field work with a Certified Home Health Agency (CHHA)
Work with a Licensed Home Care Services Agency (LHCSA)
Experience with Managed Long-Term Care (MLTC) programs
Conducting home-based patient assessments (e.g., OASIS)
Providing primary care to homebound patients
Care management for individuals receiving home and community-based services, hospice, or assisted living
Home care experience as an LPN or Home Health Aide caring for at least 2 patients per week
