Position Summary
We are seeking an energetic, relationship-focused professional to serve as our Healthcare Provider Relations & Community Outreach Coordinator. This position will play a key role in building relationships with healthcare providers and community organizations while promoting the services offered by both White Cross Pharmacy and White Cross HomeCare.
The successful candidate will develop referral relationships, educate healthcare professionals about our services, coordinate community outreach efforts, assist with healthcare marketing initiatives, and support business development activities that drive organizational growth.

Responsibilities
Develop and maintain relationships with:
- Physicians and physician practices
- Hospital discharge planners
- Case managers and social workers
- Skilled nursing facilities
- Assisted living communities
- Senior centers and community organizations
- Other healthcare referral sources
- Conduct in-person visits and presentations to referral partners.
- Build relationships with referral sources and healthcare providers to increase awareness of White Cross Pharmacy, MedPack medication management services, and White Cross HomeCare.
- Educate healthcare professionals regarding: White Cross Pharmacy and HomeCare services
- Identify opportunities to increase pharmacy transfers, MedPack enrollments, and homecare referrals.
- Represent White Cross at networking events, health fairs, professional meetings, and community outreach programs.
- Assist with marketing initiatives designed to increase brand awareness and community engagement.
- Manage and grow White Cross Pharmacy and White Cross HomeCare's social media presence through the creation of engaging content, educational posts, community outreach campaigns, and referral partner spotlights.
- Coordinate community education programs and outreach campaigns.
- Maintain records of outreach activities, referral sources, contacts, and follow-up opportunities.
- Collaborate with pharmacy and homecare leadership on growth initiatives and strategic partnerships.
- Communicate referral source and customer feedback to management.
Qualifications
Preferred Experience
- Experience in healthcare marketing, provider relations, physician relations, community outreach, referral development, homecare, pharmacy, assisted living, senior services, or business development.
- Experience working with hospitals, physician offices, homecare agencies, assisted living communities, skilled nursing facilities, or other healthcare organizations.
- Experience creating content and managing social media platforms for a business or healthcare organization.
Clinical Experience
Clinical experience is highly desirable but not required.
Candidates with backgrounds in pharmacy, nursing, medical assisting, case management, social work, patient advocacy, healthcare administration, or related healthcare fields are strongly encouraged to apply.
