Position Overview
The MDS Coordinator is responsible for overseeing the completion of Minimum Data Set (MDS) assessments, coordinating interdisciplinary care planning, and ensuring adherence to state and federal guidelines. This position is ideal for a nurse who thrives in a structured, fastpaced environment and values precision in documentation and resident assessment.
Key Responsibilities
Conduct comprehensive assessments of residents’ health status
Collaborate with interdisciplinary team members to develop individualized care plans
Ensure accurate and timely completion of all MDS assessments
Maintain thorough and compliant medical documentation and records
Utilize ICD10 and other medical coding systems for accurate reporting
Coordinate with Health Information Management teams
Implement utilization management strategies to support quality outcomes
Work within electronic health record (EHR) systems effectively
Apply knowledge of longterm care regulations and compliance standards
Required Experience & Qualifications
Active RN license with previous experience in patient care settings
Proficiency in medical coding, documentation, and ICD10
Strong understanding of the MDS assessment process
Knowledge of regulatory requirements in longterm care facilities
Ability to work efficiently in a fastpaced healthcare environment
