As the Home Health & hospice Aide you will provide personal services such as bathing, grooming and assistance with ambulation to skilled home health patients under the direction and supervision of a registered nurse or therapist. Complete personal care interventions according to the plan of care and documents accurately.
JOB SUMMARY:
Assess the psychosocial status and needs of clients. Assist the home health & hospice team in recognizing and understanding the social/mental stress and disorders that impact client health and rehabilitation.
MAJOR ACCOUNTABILITIES/ESSENTIAL JOB DUTIES:
• Provide Medical Social Services in accordance with a plan of care, including individual counseling, community resource planning, and crisis intervention.
• Assess the client’s psychosocial status
• Develop and implement a medical plan of care and revising as necessary
• Assist other team members in understanding the social and emotional factors related to the health problems
• Work with family as needed
• Prepare clinical and progress notes
• Utilize and provide client/family with appropriate community resources as needed
• Participate in discharge planning
• Consult and coordinate services with the primary care physician and other disciplines regarding changes in the client’s needs
• Participate in in-service programs
• Comply with agency policies and procedures relative to infection control and safety
