PRN Pre-Admission Nurse (LPN/RN) – Assisted Living

Location: Westlake and Middleburg Ohio, (Assisted Living Communities)
Job Type: PRN / As-Needed / Flexible Schedule $38-$48 hourly
Reports To: Regional Director

Job Summary

We are seeking a PRN Assessment Nurse (LPN or RN) to support our assisted living communities in the Cleveland market. This role is dedicated exclusively to completing pre-admission clinical assessments for prospective residents. The Assessment Nurse plays a key role in determining clinical appropriateness for assisted living residency in accordance with Ohio regulations and company guidelines.

This position does not provide direct resident care and does not involve routine staffing or scheduled nursing shifts. This role is not community based and will require travel to and from the assessment locations.

Responsibilities

• Complete comprehensive pre-admission assessments for prospective assisted living residents
• Ensure all assessments are completed within 24 hours of referral
• Determine clinical appropriateness for assisted living residency based on Ohio assisted living guidelines
• Partner with the Community Marketer to coordinate and schedule assessments
• Collect appropriate move in paperwork
• Accurately document assessment findings and recommendations
• When a clinical denial is recommended:
• Clearly document the clinical rationale
• Collaborate with the Regional Director and regional team to review and discuss the denial
• Maintain current knowledge of assisted living regulations, scope of services, and admission criteria

Schedule & Hours

• PRN / As-needed basis
• Flexible hours based on referral volume
• No guaranteed number of hours

Qualifications

• Active Ohio LPN or RN license in good standing
• Experience in assisted living, long-term care, home health, or similar setting preferred
• Strong clinical assessment and decision-making skills
• Ability to work independently and manage a variable workload
• Excellent communication and documentation skills

Additional Information

• Local travel to assisted living communities in the Cleveland area required
• Position focuses solely on assessments, pre-admission review and paperwork compliance

Pay: $38.00 - $48.00 per hour
