Responsibilities:
- Assist with transporting residents to and from medical appointments, therapy sessions, and social activities
- Provide emotional support and companionship to residents during transportation
- Monitor resident vital signs and report any changes to nursing staff
- Assist with wheelchair and stretcher transfers
- Maintain accurate records of transportation activities and resident vital signs
- Collaborate with nursing staff to develop and implement individualized care plans
- Participate in orientation and training programs to ensure compliance with facility policies and procedures
