Western Tidewater CSB is seeking a Case Manager for our Early Intervention Program (age group, birth-3yrs) in Suffolk, VA.

- Consumer assessment
- Service linkage
- Coordination and development
- implementation of a case management ISP
- Assist and provide follow up instruction
- Parent education
- Monitor service delivery through contacts with consumers receiving services, family members, service providers, and periodic site/home visits
- Manage consumer records according to all Licensure and Medicaid standards
- BA/BS degree from an accredited college or university with major course experience in Psychology, Social Work, or related field
- One year of clinical experience in a Human Services field
- Early Intervention Certification preferred
- Valid driver’s license with acceptable DMV driving record at the time of employment and continuously, thereafter
