We are seeking Qualified Mental Health Professionals for our

______Waverly Nursing Home Pilot Program______________ department.

**Gero-Psychiatric, Mental Health, or ID/DD experience is preferred for this position. **

Responsibilities and Duties for QMHP:

• Teach consumers appropriate strategies to manage behavioral and mental health symptoms
• Assist with consumer service plans related to identified needs and goals
• Assist consumers in gaining access to appropriate medical, psychiatric, social, educational, vocational and other support and services
• Provide consumer training/reinforcement, linkage to supports/services
• Advocate on behalf of consumer, family and providers
• Monitor/assess service delivery
• Behavior management and crisis intervention.

Qualifications and Skills:

• Bachelor’s Degree in a Human Service field
• One year experience working directly with individuals with intellectual disabilities, mental illness, substance abuse or who are dually diagnosed.
• QMHP-C or QMHP-A certified by the Board of Health Professions
• Valid Driver’s License with acceptable DMV record.
