GENERAL STATEMENT OF DUTIES 

The CPSP/Perinatal Program Manager provides operational oversight of Western Sierra Medical Clinic's Comprehensive Perinatal Services Program (CPSP), ensuring consistent implementation, coordination, monitoring, and continuous improvement of program workflows that support pregnant and postpartum Medi-Cal patients. This position oversees daily program operations, staff coordination, workflow performance, quality monitoring, documentation compliance, Community Health Worker utilization, Healow Baby and Child engagement, eClinicalWorks (eCW) workflows, and collaboration with internal departments and external partners to ensure timely, coordinated, and compliant perinatal care.

 

The role requires strong leadership, project management, clinical operations, quality improvement, regulatory compliance, and change management skills. The Manager works closely with providers, clinical leadership, care coordination, quality, billing, IT, and community partners to improve program performance, remove operational barriers, support staff accountability, strengthen patient access and continuity of care, and ensure the CPSP program meets organizational, regulatory, and payer requirements.

 

GENERAL DUTIES

  1. Develop and manage the CPSP implementation and maintenance workplan, including milestones, assigned owners, timelines, risks, dependencies, and reporting cadence.
  2. Coordinate CPSP enrollment, orientation, assessments, reassessments, individualized care plans, interventions, referrals, case coordination, postpartum follow-up, and documentation workflows.
  3. Ensure the required CPSP client orientation is completed and documented, including explanation of available CPSP services, voluntary participation, patient rights, confidentiality, how services support pregnancy and postpartum care, and the patient’s agreement or declination to participate.
  4. Ensure CPSP services are aligned with California Medi-Cal requirements, CDPH/MCAH guidance, Partnership HealthPlan expectations, and WSMC clinical operations.
  5. Partner with midwives, OB/GYNs, FP/OBs, behavioral health, nutrition, dental, pediatrics, lactation, referrals, billing, and quality teams to support comprehensive perinatal care.
  6. Appropriately engage the Community Health Worker team to support patient outreach, appointment reminders, transportation and resource navigation, health education reinforcement, warm handoffs, social needs follow-up, and closure of non-clinical barriers affecting prenatal and postpartum care.
  7. Build and maintain eClinicalWorks workflows, templates, order sets, referral pathways, tracking tools, registries, tasking processes, and documentation standards for CPSP.
  8. Coordinate Healow component management, including appropriate use of Healow Baby and Child tools to support prenatal engagement, postpartum communication, newborn/well-baby reminders, patient education, care coordination, and follow-up through two months postpartum.
  9. Coordinate postpartum and well-baby support through two months postpartum/60 days following delivery, including maternal recovery, breastfeeding/lactation support, newborn appointment reminders, pediatric handoff, newborn screening education, newborn hearing screening awareness, and connection to infant care resources.
  10. Monitor prenatal patient attribution, Medi-Cal eligibility, Partnership HealthPlan enrollment, CPSP eligibility, missed opportunities, and completion of required CPSP elements.
  11. Support accurate charge capture, coding readiness, documentation completeness, and audit preparedness for CPSP services.
  12. Coordinate internal and external referrals, including WIC, dental, behavioral health, nutrition, genetic screening, doula services when available, lactation support, family planning, pediatric care, transportation, social services, and community-based supports.
  13. Maintain CPSP referral pathways and resource lists that include, as applicable, WIC, Medi-Cal Dental, Family PACT/family planning, CHDP or child wellness services, Genetic Disease Screening Program resources, newborn screening and newborn hearing screening information, lactation/breastfeeding supports, behavioral health, nutrition, transportation, and community-based social service resources.
  14. Develop training materials, job aids, checklists, standing workflows, and staff competency tools for CPSP-related roles.
  15. Maintain site-specific CPSP protocols, staff resources, assessment tools, care plan workflows, required referral processes, documentation standards, and patient education materials consistent with CDPH/MCAH expectations and WSMC operations.
  16. Prepare routine status reports for clinic leadership, including enrollment volume, assessment completion, trimester reassessment rates, postpartum completion, referral closure, claim readiness, and program barriers.

Supervise or coordinate assigned CPSP/perinatal staff workflows, monitor daily operational performance, review dashboard trends, ensure timely completion of CHW-supported and Healow-supported activities, and escalate barriers requiring leadership action.

Qualifications:

MINIMUM QUALIFICATIONS

  • Bachelor’s degree in public health, nursing, healthcare administration, social work, health education, nutrition, business, or a related field preferred; equivalent relevant experience may be considered.
  • Strong project management skills, including workplan development, milestone tracking, meeting facilitation, barrier resolution, and cross-functional follow-up.
  • Knowledge of Medi-Cal, managed care, health equity, rural healthcare access, and perinatal support services preferred.
  • Ability to communicate effectively with clinicians, patients, operational teams, billing staff, community partners, and health plan representatives.
  • Strong attention to documentation, compliance, confidentiality, patient engagement, and workflow reliability.

 

DESIRED EXPERIENCE

  • Supervisory, project management, quality improvement, care coordination, maternal-child health, or FQHC operations experience preferred.
  • Experience using eClinicalWorks or another electronic health record system strongly preferred.
  • Experience supervising staff or coordinating team workflows, leading implementation projects, analyzing operational dashboards, training multidisciplinary teams, and managing recurring quality or compliance review processes.

Salary : $79,040 - $89,840
