Ability to earn more working a 5 day work week!

Performance based quarterly bonuses after 6 months (up to $2,000 quarterly)

Key Responsibilities:
The Nurse Practitioner will work in outpatient clinics managing chronic medical conditions as well as providing care in the outpatient dialysis clinics.

Key Responsibilities:

  • Conduct comprehensive history and physical assessments accurate and relevant to each patient's age, gender, and clinical problems.
  • Order, interpret, and evaluate diagnostic studies, laboratory tests, and renal imaging.
  • Develop, implement, and adjust evidence-based treatment plans, including prescribing medications and managing dialysis therapy regimens.
  • Round on patients in dialysis facilities and provide consultative care in both ambulatory and inpatient settings.
  • Coordinate care and make appropriate referrals to external specialists, including urology, transplant, and imaging providers.
  • Serve as a advocate and educator for patients and their families, guiding them through kidney disease progression and treatment options.
  • Collaborate effectively with physicians, health professionals, and community agencies to ensure seamless care across settings.
  • Participate in departmental meetings, committees, quality improvement programs, and research projects.
  • Maintain accurate, complete, and timely electronic medical record documentation (charts locked within 48 hours).

Basic Qualifications/REQUIRED:

  • 2 years of experience in care of adults with chronic diseases or family medicine
  • Advance Nurse Practitioner education (ANP)
  • Valid Colorado license a Nurse Practitioner on the Colorado Advanced Practice Registry. (or ability to get registered prior to start date)
  • Full Prescriptive Authority in the State of Colorado. (or ability to obtain this prior to start date)

Preferred Qualifications:

  • Clinic and dialysis experience highly desirable
  • Nephrology, internal medicine or internal medicine subspecialty
