Overview: Western Home Communities is seeking a passionate and dedicated Monday - Friday Clinical Services Coordinator (Registered Nurse) to join our Assisted Living team at our Jesup community. We are looking for a detail-oriented, compassionate individual with a strong desire to create fulfilling lifestyles for our residents. In this role, you will have the opportunity to provide high-quality nursing care while making a meaningful impact on the lives of those you serve. Key Responsibilities:
- Resident Care Management: Guide and direct resident care as part of the management team, ensuring all residents under your care receive the appropriate services and attention.
- Individualized Care Plans: Create and maintain personalized service plans for each resident based on assessments and reviews of their needs.
- Effective Communication: Coordinate with physicians and the immediate supervisor to communicate resident needs and treatment plans.
- Admission Coordination: Assist in the preparation and planning of resident admissions, collaborating with family members and others to ensure comprehensive nursing care.
- Emotional and Physical Support: Provide comfort and safety to residents, offering both emotional and physical support, and extending support to families.
- Team Leadership: Supervise, consult with, and teach Resident Assistants, CNAs, and Medication Managers to ensure effective team performance.
Qualifications:
- Graduate of a state board-approved RN school
- Currently licensed as an RN in Iowa
- CPR certification (or willingness to obtain)
- Strong attendance and reliability
- Ability to work well in a team and foster collaborative relationships
