General Statement of Duties:

The Western CT Area Agency on Aging (WCAAA) is dedicated to promoting the independence, health, and quality of life for older adults and individuals with disabilities in our region. We provide a range of services and supports, including comprehensive care management, to help individuals remain safely and comfortably in their own homes and avoid institutionalization. Job Summary:

The Care Manager is a full time 35 hour a week position and plays a crucial role in risk mitigation and the prevention of institutionalization for seniors and younger individuals with disabilities through the provision of comprehensive care management services. Daily tasks involve conducting thorough in-home assessments to identify individual strengths and areas of need. The Care Manager is responsible for the development and implementation of individualized care plans and the coordination of services to support individuals in remaining safely in their homes. This position provides ongoing monitoring, advocacy, and support within an atmosphere of person-centered planning and acts as the primary service contact with the client and their families.

Responsibilities:

  • Conduct comprehensive in-home assessments of seniors and younger individuals with disabilities to identify their strengths, needs, and preferences.
  • Develop individualized, person-centered care plans with measurable goals to address identified needs and promote independence.
  • Implement and coordinate services outlined in the care plan, linking clients with appropriate community resources, supports, and providers.
  • Provide ongoing monitoring of service delivery and client well-being, making necessary adjustments to the care plan in a timely manner.
  • Serve as the primary point of contact for clients and their families, providing ongoing support, education, and advocacy.
  • Assist clients and their families with required paperwork to secure and maintain essential benefits, including but not limited to Medicaid, Medicare, housing assistance, and transportation services.
  • Participate actively in various interdisciplinary team meetings concerning the individual’s well-being, service plan, and any status changes, which may require travel beyond the WCAAA region.
  • Maintain accurate and timely documentation of all assessments, care plans, service coordination activities, and client interactions in accordance with agency policies and procedures.
  • Adhere to all agency policies, procedures, and ethical guidelines.
  • Collaborate effectively with other WCAAA staff, community partners, and service providers.
  • Assess individuals in their homes, nursing facilities, or other skilled facilities as needed.

Qualifications:

  • Candidates must be willing to work on-site from the Waterbury office during the initial 6-month period, in between field work assignments.
  • Remote work is available to eligible employees after successful completion of the initial 6-month on-site period and demonstrated skill proficiency following the training curriculum.
  • Minimum Education and Licensure Requirements:
  • Bachelor’s or Master’s degree in Social Work or a related field.
  • Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) preferred.
  • OR a Bachelor’s degree in Nursing with a current active Connecticut Registered Nurse (RN) license.
  • All applicants must have a minimum of two (2) years of demonstrated related experience working in health and human services.
  • Specific Requirement for Acquired Brain Injury (ABI) Care Managers: Must possess an LMSW, LCSW, or a Master’s degree in Human Services, Counseling, or Rehabilitation Counseling, in addition to two (2) years of demonstrated related experience working with individuals with acquired brain injuries.
  • Must possess strong organizational skills and excellent time management abilities.
  • Demonstrated professional interpersonal and communication skills (both verbal and written).
  • Knowledge of community resources available to older adults and individuals with disabilities in Western Connecticut.
  • Ability to conduct comprehensive assessments in various settings, including client homes and facilities.
  • Familiarity with computer systems and the ability to utilize electronic documentation effectively.
  • Valid Connecticut driver’s license and reliable vehicle are required as this position necessitates travel within the WCAAA service area.
  • Second language proficiency is a plus.
