What are the responsibilities and job description for the Connection Specialist, JCCA - Westchester County, NY position at Westchester Jewish Community Services (WJCS)?
Isolation to Connection assists isolated older adults by mobilizing the community resources and necessary linkages to help meet their emotional, social, and physical needs. A part-time “Connection Specialist” hired by the Jewish Community Center of Mid-Westchester will help identify isolated older adults and will successfully connect them with programs and services. Through extensive outreach, the Connections Specialist will also build strong relationships with the full continuum of older adult service providers, government agencies, law enforcement, health care providers, not for profit agencies, and faith-based organizations. The Connection Specialist listens to the needs expressed by the older adult, identifies meaningful services or programs, navigates complex systems, advocates on behalf of the older adult to provide a referral avenue that skillfully connects the older adult with necessary services and programs, reducing isolation and loneliness. The part-time Connections Specialist will receive ongoing clinical supervision from Westchester Jewish Community Services.
- Part-time, 18 hours per week, $30/hour.
- Social Service professional, LMSW/LCSW preferred.
- Primary job function involves telephone contact with older adults, their family members, various elder care providers, social service agencies, medical offices, as well as community and faith-based organizations.
- Knowledgeable in work with older adults aging in community.
- Experience in telephone support and mental health triage services.
- Familiar with Westchester County, New York’s varied services for older adults, social and community services.
- Telephone based work may be done remotely if preferred.
- Excellent communication skills.
- Outstanding organizational skills as the program requires specific metrics and intake processes.
- Connections Specialists need to be able to plan and present community outreach and education presentations.
- Connections Specialists will need to travel to occasional community presentations. Access to transportation will be necessary.
