What are the responsibilities and job description for the Patient Navigator position at Westchester Community Health Center?
Job Summary:
Under the general supervision of the Director of Medical Case Management, the patient navigator will perform all duties required below and report to the program supervisor.
Primary Responsibility of the Position:
- Case findings
- Reassessment
- Self-management assessment
- Client assistance
- Client engagement
- Outreach for client reengagement
- Linkage to services
- Accompaniment
- Health Education
- Complies and maintains necessary reports, records, and statistics as required by Public Health Solutions and NYC DOHMH
- Performs other related duties as assigned.
Education and Experience:
- Bachelor’s Degree
- At least 2 years of Case management experience
- Must have a valid NYS driver's license.
- Excellent writing skills
- Proficiency in Microsoft Excel, word, and PowerPoint is highly desired.
Job Type: Full-time
Pay: $40,000.00 per year
