What are the responsibilities and job description for the Case Manager - MAT position at WESTCARE NEVADA INC?
Position Summary:
“This position is assigned in direct support of the Medication Assisted Treatment (MAT) Mobile program and will contribute to its day-to-day operations, client engagement, and service delivery”.
Person in this position will work in collaboration with a mental health counselor to provide mental health services to patients/clients of WestCare who have been diagnosed with a substance abuse disorder, a mental illness or both. Essential Job Functions include those listed below. Other duties may be assigned as needed.
Essential Job Functions:
- Assess resource needs of patients.
- Educate and refer patients to community agencies and businesses that appropriately address the patients’ needs for food, shelter, clothing, employment, transportation, etc.
- Assist patients with establishing or re-establishing support systems within their community.
- Act as advocate on behalf of the patient to improve access to needed services.
- Participate in clinical staffing decisions regarding the patients’ current strengths and resources and their need for additional resources.
- Complete documentation of services on provided forms in a timely manner.
- Provides input into the patient’s treatment needs and disposition.
- Assists other WestCare staff as needed in a collaborative manner to promote cooperation and portray a professional image to the community.

Education:
- A minimum of a bachelor’s degree in social work; or a Bachelor’s degree with a social work major; or working towards a degree in social work; or comparable years of qualifying experience
Experience and Competencies:
- Ability to work well in a team environment.
- Ability to exercise good judgment and discretion.
- Ability to operate a computer and complete documents in Microsoft Word program formats or the ability to learn.
- Ability to respond as needed to crisis situations in an efficient manner.
- Ability to complete work tasks within scheduled work hours.
- Excellent verbal and written communication skills.
- Willing to seek out new work projects and contribute ideas and efforts to the constant improvement of WestCare services.
Salary : $24 - $25
