POSITION SUMMARY:
The Licensed Clinician is responsible for providing clinical oversight, behavioral health assessment, and care coordination support for individuals participating in CalAIM Pre and post release services. The clinician ensures that behavioral health needs are identified prior to release and that appropriate treatment, mental health services, and community supports are incorporated into the individual’s care plan to support successful reentry. Essential Job Functions are below.
ESSENTIAL JOB FUNCTIONS:
• Provide clinical consultation and guidance to care coordination staff regarding behavioral health needs and appropriate service referrals.
• Assist in the development of individualized care plans addressing mental health, substance use, medical, and social determinants of health.
• Support care coordination efforts by identifying appropriate behavioral health, substance use treatment, and community-based resources.
• Ensure participants with behavioral health needs are connected to appropriate treatment providers prior to release.
• Provide clinical review of participant cases to ensure services are appropriate and responsive to identified needs.
• Collaborate with correctional health staff, community providers, and Managed Care Plans to support continuity of care following release.
• Support warm handoffs to behavioral health providers and treatment programs in the community.
• Ensure services and documentation comply with CalAIM, Medi-Cal, and program requirements.
• Participate in case conferences, multidisciplinary meetings, and program planning activities.
• Provide guidance to staff on trauma-informed care, behavioral health engagement strategies, and working with justice-involved populations.

ESSENTIAL QUALIFICATIONS:
To perform this job successfully, an individual must be able to perform the duties and responsibilities satisfactorily. The requirements listed below are representative of the knowledge, skill, and/or ability required.
Experience and Competencies:
• Knowledge of Medi-Cal, CalAIM, or care coordination models preferred.
• Valid driver’s license and ability to obtain and maintain WestCare’s vehicle insurance is required.
• Ability to obtain and maintain successful background clearance.
• During your tenure with WestCare there may be new requirements, including, but not limited to vaccinations that are issued by local, State, Federal, and/or Funders that WestCare may have to comply with. Should this occur Human Resources or appropriate personnel will inform you.
• CPR Certification, First Aid Certification, and an annual Tuberculosis test.
EDUCATION and/or EXPERIENCE:
• Experience working with individuals with behavioral health and/or substance use disorders.
• Experience working with justice-involved populations or reentry services preferred.
• Master’s degree in Social Work, Marriage and Family Therapy, Psychology, Counseling, or a related behavioral health field.
LICENSURE/CERTIFICATION:
• Active California licensure as an LCSW, LMFT, LPCC, or Clinical Psychologist in good standing.
WORKING CONDITIONS:
• Work is performed in a combination of correctional facilities, office environments, and community settings.
• Regular interaction with justice-involved individuals and multidisciplinary care teams.
• Local travel is required to correctional facilities, partner agencies, and community providers.
Salary : $90,000 - $110,000
