Position responsible for direct, basic, and supportive care to Consumers within a program.
Essential Duties and Responsibilities include the following. Other duties may be assigned to meet Consumer and agency needs.
• Provides orientation to program, i.e.: rules and regulations, expectations.
• Provides an outlined daily schedule of activities.
• Provides transportation to and from appointments outside the residence.
• Provides oversight and intervention to consumers to ensure a safe and cooperative recovery environment.
• Attends and supervises all outside meetings and approves extracurricular outings, enforcing program rules.
• Records a detailed account of the consumers behavioral, medical and staff interactions in the daily shift notes.
• Maintains EMR and scans documentation into appropriate headers.
• Assists with cooking, laundry, and other general household activities necessary to maintain sanitary conditions and general housekeeping upkeep.
• Offers a general account of the ending shift to the employees coming onto shift to ensure a smooth transition of care.
• Other duties may be assigned to meet consumer and agency needs.
• Complies with federal, state and company policies, procedures, and regulations.
• Maintains confidentiality of all information and complies with HIPAA regulations.
• Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards and code of ethical behavior.
Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. Mobility and ability to stand and walk for long periods, bend, reach and lift up to 50 pounds are required in this position.

Must work well in a team environment with staff, consumers, and others in a professional, cooperative, ethical, respectful and effective manner.
Education and/or Experience: Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen. Must have a valid driver's license and current auto insurance.
