Physical Requirements: The employee is frequently required to walk; use hands to finger, handle, or feel; and reach forward with hands and arms. The employee is occasionally required to sit and stoop, kneel, or crouch. The employee must occasionally lift and/or move up to 50 pounds.

- Provides on-the-job training in work- and work-related skills
- Provides training in skills essential to obtain and retain employment, such as the effective use of community resources
- Implements work-readiness assessments as determined by the Division of Rehabilitation Services
- Serves as a job coach to provide supervision and monitoring to employed individuals
- Provides transportation as needed for employment and employment related tasks
- Provides support and utilizes approved intervention methods to assist individuals who may be experiencing maladaptive behaviors.
- Serves as a member of the individual's team and facilitates advocacy, as needed.
- Maintains detailed daily records of services provided
- Documents individual progress towards defined employment goals consistent with individual's treatment plan and/or DRS Individualized Plan for Employment
- Monitors individual safety, including physical, medical, and mental health needs
- Attends trainings and meetings required by the Division of Rehabilitation Services
- Other duties may be assigned to meet individual and agency needs
- Complies with federal, state, and company policies, procedures, and regulations.
- Maintains confidentiality of all information and complies with HIPAA regulations.
- Demonstrates knowledge of and supports Westbrook Health Services mission, vision, values, policies and procedures, operating instructions, confidentiality standards, and code of ethical behavior

Qualification Requirements
- Works well in a team environment
- Work with others in a professional, cooperative, ethical, respectful, and effective manner.
- Has a valid driver’s license and current auto insurance.
- Must pass Criminal Investigation Background and DHHR background checks as well as post offer drug screen.
