WESTARM Home Health is looking for an part-time or PRN registered nurse (RN) to provide home health services in the Lower Burrell, New Kensington, Leechburg, Apollo, Plum, Kittanning, Sarver and surrounding areas. This is a great opportunity for a RN that enjoys connecting with patients and having independence in their work while working for a small and local company. You will be able to make your daily schedule. One on-call weekend required per month. This position would likely be for 2-3 days per week.
PRN evaluations and regular visits are paid at competitive rates. Weekend visits are paid at a higher rate.
Responsibilities:
Primary functions are to plan, develop, implement, evaluate, organize and direct home care services with the interdisciplinary team including physician, patient/family, and administer skilled nursing care for clients in their place of residence and assume the responsibility for the total provision of care.
Essential Duties and Responsibilities:
- Assesses home health patients to identify the physical, psychosocial, and environmental needs as evidenced by documentation, clinical record, case conference, team report, and evaluations
- Case manages and provides clinical oversight and direction to the interdisciplinary team, physician, and family
- Implements/develops/documents the plan of care and revises as necessary
- Assures continuity of quality patient care delivered with appropriate documentation
- Monitors assigned cases to ensure compliance with requirements of third-party payors
- Provides effective communication to patient/family, team members, physicians, and other health care professionals
- Adheres to all company policies and procedures.
Minimum Qualifications (for RN):
- Graduate of an accredited college/university, associate, diploma, or baccalaureate school of nursing
- Current State License as a registered nurse or licensed practical nurse
- Current Driver's License
- Current CPR certification and 2-Step TB test
- Two years of experience as a registered nurse in a clinical care setting preferred, home health experience preferred.
Pay: $55.00 - $75.00 per hour
