What are the responsibilities and job description for the Licensed Vocational Nurse position at West Texas Healthcare LLC?
Licensed Vocational Nurse will provide direct patient care to home bound patients.
Responsibilities and Duties:
- Provides wound care, disease process education, medication administration, and other clinical duties as prescribed by NP's, PA's, and Physicians.
- Carrys out the clinical plans of care to assist patients maintain their independence.
- Coordinates with the interdisciplinary clinical team regarding patient changes in condition and proactively communicates patient's clinical needs.
- Provides education to patients and families on proper home health care procedures and strategies.
- Advocates for appropriate patient care and recommends strategies to help improve the patient's quality of life.
Qualifications and Skills:
- Current Texas LVN License.
- Current Certification in BLS/CPR
- Valid Texas Driver's License and Automobile Insurance
- Knowledge of WellSky EMR software is a plus
