Reports to: Clinical Manager
Essential Job Functions and Responsibilities:
- Provides physical therapy services to patients according to a written physician's plan of care period
- Follow conditions of participation
- Work within the scope of Physical Therapy
- Coordination of patient care
- Performs other duties as assigned
Position Qualifications:
- Licensed to practice as a physical therapist with through the state
- Preferred 2 years of experience as a Physical Therapist
- No Home Health experience is required
- Possesses and maintains current CPR Certification
- Driver License and Auto Insurance is required
