Job Description
The Resident Care Coordinator plays a pivotal role in our team's success and the well-being of our residents. This position is responsible for collaborating with the Director of Nursing to develop, direct, and implement resident care services in accordance with federal, state, and local standards.
Key Responsibilities:
- Develop, maintain, and enforce facility and state-specific policies and procedures governing daily nursing service functions.
- Maintain current references on written nursing materials to support the nursing service department's day-to-day needs.
- Communicate nursing services schedules with other departments to ensure continuity of resident care.
- Assist in developing and implementing a Continuous Quality Improvement program for the nursing services department.
Nursing Care Functions:
- Conduct daily nursing functions in accordance with current rules, regulations, and guidelines.
- Ensure all nursing personnel comply with written policies and procedures.
- Admit, transfer, and discharge residents with accurate documentation.
- Participate in new resident/family member orientations and communicate effectively with healthcare providers.
- Evaluate resident care and report significant events during daily clinical rounds.
Requirements:
- Minimum 1 year of experience in nursing, preferably in long-term care.
- Experience in supervision and teaching principles.
- Current, unencumbered active license to practice as a licensed nurse in this state.
