JOB OPPORTUNITY
The Community Health Worker (CHW) works primarily with patients in the CalAIM program under supervision of the CalAIM Care Manager, providing direct services and education to patients, building trusted relationships with the individuals they serve to improve health and well-being over time. Specifically, the CHW will work to improve evidence-based medical and behavioral health integration practices within primary care, specialty care, and behavioral health.
Position is a full-time, on-site role located at our Third Street House in Guerneville, California.
DUTIES
- Provides health coaching with provider guidance, patient health care and resource system navigation, health education and/or health promotion for patients within assigned health program.
- Connects patients with supportive services and community resources that are critical to their overall health and wellness.
- Provides Care Management and Care Coordination including engaging families, providing home visits, appointment advocacy, and addressing basic needs.
- Assists with Care Management for Complex Care Patients including supporting hospital transition, barrier reduction, health coaching, and system coordination.
- Conduct daily data entry within the electronic patient record to track patient encounters and progress.
- Conduct extensive outreach activities.
EXPERIENCE
- Certificate of completion from Community Health Worker Program or equivalent experience.
- 1-2 years’ experience working with patient population within project area, ideally within a healthcare or community setting.
- Knowledge of community resources and networking ability.
- Willingness to go into the community, including encampments.
- Must have an acute attention to detail and follow through on commitments.
- Strong organizational skills and the ability to multitask effectively in a busy environment.
- Expected to have strong interpersonal skills and can understand and follow written/verbal instructions.
- Ability to work independently with little or no supervision.
- Excellent interpersonal skills to interact positively with patients and staff alike.
- Proficiency in using office software and electronic health record systems is beneficial.
PHYSICAL REQUIREMENTS
- Ability to move freely about the clinic/office and carry light loads (up to 25 lbs.)
- Prolonged standing and /or sitting required.
- Ability to interface with electronic office equipment for extended periods of time.
