We are currently seeking a part time Clinical Case Manager to work 25 flexible hours, Monday - Friday.

The Clinical Case Manager on the Partial Care Treatment Team provides supportive clinical and case management services to the young adult and adult consumers with psychiatric and co-occurring disorders.

Case manager duties include assessing, planning, implementing, monitoring and evaluating actions required to meet the client’s health and human services needed. They maintain a caseload of consumers to include EMR documentation as needed, provide supportive individual and family counseling, facilitate and co-facilitate psycho educational groups and activities and provide basic clinical case management services.
Additionally required, meeting of documentation standards and required clinical competencies as identified; Basic computer/keyboarding skills, including familiarity with word processing programs, (MS Word preferred) for report writing, forms, records, letters, ability to learn and use the electronic client recordkeeping system, etc., additional requirements may occur as specific job needs evolve.
F*lexible Qualifications include Bachelor's Degree in a human services field from an accredited school, or* an associate’s degree and two years’ experience in providing human services, or five years of human service experience, or possess a relevant professional credential, such as Certified Psychiatric Rehabilitation Practitioner, Bachelor’s degree in Social Work, Certified Rehabilitation Counselor, Certified Alcohol and Drug counselor, Bachelor’s degree in Rehabilitation, or Bachelor of Science in Nursing. CPR/First Aid trained, to be obtained prior to delivering services alone and within 30 days of hire.
Preferred Qualifications: Same as above, plus prior experience with adult psychiatric population. One-year experience as mental health worker in a Partial Care setting. Valid driver’s license. Good driving record, meets recommendations of Agency Automotive Insurer.
