Position Purpose

The Registered Nurse (RN) for the Community-Based Adult Services (CBAS) program will be an experienced professional who enjoys working with older adults and individuals with disabilities. As a key member of a multidisciplinary team—comprising social workers, program aides, physical therapists, occupational therapists, speech therapists, and dietitians—the RN collaborates to assess participants and maintain their community involvement, aiming to prevent unnecessary hospitalizations and nursing home placements. Reporting to the Adult Day Health Care (ADHC) Program Director, the RN coordinates all health-related activities, determines nursing and personal care needs, interfaces with primary care providers and caregivers, and provides health education to staff, participants, and family members.

Principal Responsibilities

  • Develop, oversee, and implement all nursing procedures in compliance with regulatory standards and under the direction of the Center Director and licensing requirements.
  • Conduct initial assessments of participants to determine nursing and personal care needs prior to enrollment, documented and signed by the RN.
  • Create, implement, and monitor individualized care plans that outline short- and long-term nursing goals, including vital sign monitoring, nutrition, hygiene, and continence management.
  • Reassess and document nursing and personal care services regularly, with all services dated and signed by the RN or staff providing care, including notes on treatments and participant responses.
  • Collaborate with primary care physicians and the ADHC Medical Director on participant assessments, admissions, and any significant health changes.
  • Oversee that all participants of ADHC/ADC have tuberculosis test or chest x-ray.
  • Interface with ADHC Medical Director and/or PMD in initial assessment and ongoing treatment of participant.
  • Monitor participants’ blood pressure, pulse, and weight monthly or as needed, noting and reporting any significant changes to family members and primary care providers.
  • Prepare quarterly progress notes on nursing and personal care services, signed by the RN.
  • Address participant, staff, or volunteer concerns and report any issues to the Center Director.
  • Support research activities related to program evaluation and nursing components.
  • Oversee medication administration and documentation as specified by ADHC certification.
  • Develop and manage a continence-training program for participants, ensuring compliance with licensing requirements.
  • Supervise program aides in implementing the nursing care plan and physical/occupational therapy maintenance.
  • Educate participants and families on health-related issues, counseling them as needed.
  • Execute emergency protocols, including arranging transportation if necessary.
  • Deliver supportive and restorative nursing care per the care plan and provide ongoing health status monitoring.
  • Train participants in self-administration of medications when appropriate.
  • Collaborate with the dietitian to adjust menus for participants with special dietary needs, such as low-sodium or low-cholesterol diets.
  • Adhere to all organizational safety policies, procedures, and regulatory requirements to ensure a safe environment for patients, staff, and visitors.
  • Participate in required safety and compliance trainings and apply learned practices in daily work activities.
  • Identify, report, and help mitigate unsafe conditions, incidents, or hazards in the workplace.
  • Use equipment, tools, and supplies safely and responsibly in accordance with organizational and regulatory standards.
  • Maintain awareness of infection control, occupational health, and emergency preparedness procedures appropriate to the role.
  • Perform other duties as assigned.

Requirements

  • Associate's Degree (A.A.) or Bachelor’s Degree in Nursing.
  • Valid RN license issued by the California Board of Registered Nursing.
  • CPR/BLS certification from American Heart Association
  • Minimum of two years’ experience in the aging field, such as in Adult Day Health Care, Nursing Homes, Residential Care Facilities, or Gerontology treatment programs.
  • Preferred experience in administrative or teaching roles.
  • Strong understanding of issues affecting frail elderly individuals and their families.
  • Record keeping and assessment experience, with attention to accuracy and detail.
  • Knowledge of regulatory standards related to elder care and patient safety.
  • Bilingual Ability (Spanish/English) preferred.
  • Knowledge of word processing, data base, email, Internet, Electronic Health Record, and spreadsheet software/applications.

Physical And Environmental Requirements

  • Physical Stamina-Ability to stand, walk, and lift participants as needed, especially during emergencies or when assisting with personal care.
  • Ability to Work in a Community Setting-Comfortable working in non-traditional healthcare environments, such as day centers or homes.
  • Infection Control Knowledge-Understanding and adherence to infection control protocols and procedures.
