Essential Functions
- Coordinate the transfer of patients within the hospital or between healthcare facilities.
- Serve as the primary point of contact for communication with sending and receiving facilities.
- Conduct patient assessments to determine the clinical status and medical needs before transfer.
- Ensure that necessary medical documentation and patient records accompany the transfer.
- Complete and maintain accurate documentation related to patient transfers, including medical records, transfer summaries, and communication logs.
- Coordinate the allocation of necessary resources for patient transfers, including appropriate medical personnel, equipment, and transportation.
- Provide support and information to patients and their families regarding the transfer process.
- Address concerns and ensure that patients and families are informed and comfortable with the transfer plan.
