What You’ll Do
- Provide case management and support services for children and youth up to age 21.
- Collaborate with families, schools, and community partners to ensure comprehensive care.
- Implement person-centered plans and monitor progress toward goals.
- Maintain accurate documentation and comply with agency standards.
Qualifications
- Education: Bachelor’s Degree (BA/BS).
- Complete MDHHS Wraparound training.
- Have a criminal history screen, including state and local child protection agency registries.
- Be supervised by an individual who meets the criteria as a Child Mental Health Professional (CHMP) who has completed MDHHS required training.
Skills & Abilities
- Proficiency in Microsoft Office Suite and standard office technology.
- Strong communication, organization, and time management skills.
- Critical thinking and problem-solving abilities.
- Attention to detail and accuracy in documentation.
Certifications Required
- Emergency Services (E.S.) Certification
- CAFAS Certification
- Wraparound Certification
Other Requirements
- Valid Michigan driver’s license, vehicle insurance, and good driving record.
- Ability to work flexible hours and travel within service area.
- Must pass pre-employment background checks (criminal, driving, education, employment).
