Role Description
This is a full-time, on-site Care Coordinator role located in Griffin, GA. The Care Coordinator will be responsible for managing patient transitions and care plans to ensure quality outcomes. Daily responsibilities include collaborating with patients, families, and healthcare providers to coordinate services, monitoring patient progress, facilitating communication between care teams, ensuring access to community resources, and advocating for patient needs. The role involves comprehensive case management and support to promote patient-centered care.
Qualifications
- Strong planning, coordination, and organizational skills for managing patient care and transitions.
- Proficiency in communication, problem-solving, and collaboration with diverse groups of patients, families, and healthcare professionals.
- Knowledge of healthcare systems, community resources, case management best practices, and patient advocacy.
- Experience working with electronic health records (EHR) and administrative healthcare systems.
- Empathy, cultural competence, and the ability to build trust with patients from various backgrounds.
- Previous care coordination, case management, or healthcare experience is an advantage.
Education Minimum Requirements
Bachelor’s Social Work – Required
Master’s Social – Preferred
Minimum Experience Requirements
Minimum 1 year experience in healthcare in the acute care setting, related field or skilled care environment or community or educational internship in care coordination.
A background in medical social work in an acute care setting. Strongly preferred.
Minimum License/Certifications Requirements
Basic Life Support through the American Heart Association or American Red Cross or Basic Life Support Instructor.
Additional License/Certification Preferred
LCSW – Licensed Clinical Social Worker GA
LMSW – Licensed Master Social Worker GA
