Job Summary

We are seeking a skilled Physical Therapist to provide essential therapy services to patients in their homes. The ideal candidate will work with individuals facing physical impairments, functional limitations, or disabilities stemming from various causes, including injury or disease. Our patient demographic ranges from infants to seniors over 100 years of age.

• Deliver personalized physical therapy services to patients in their residence.


• Collaborate effectively with patients, their families, physicians, and other healthcare professionals to assess patient needs.


• Coordinate and implement therapy services to meet the specific requirements of each patient.


• Maintain accurate and timely electronic documentation that adheres to agency policies and standards of practice.


• Ensure interdisciplinary communication to enhance patient care and outcomes.

• Must hold a valid Physical Therapist Professional License in Massachusetts.


• Minimum of 1 year of experience in home health care.


• At least 1 year of travel experience as a therapist.


• Ability to travel to various cases as assigned.

• Strong assessment and clinical skills to effectively evaluate and treat patients.


• Excellent communication skills to collaborate with medical professionals and families.


• Proficiency in electronic medical records and documentation.


• Compassionate and patient-centered approach to care.
