Wellspring Hospice is seeking a full-time, salaried RN Case Manager with hospice experience to serve our Northern territory - mainly the Lorain, Avon, Amherst and Wellington areas. In this role, you will manage a patient caseload and provide compassionate nursing care in patients’ homes and care facilities. Responsibilities include comprehensive assessments, individualized care planning, pain and symptom management, patient and caregiver education, timely clinical documentation, and hospice aide supervision. You will collaborate with physicians and our interdisciplinary team to support patient comfort, dignity, and quality of life, and participate in the on-call rotation.
Qualified candidates must be graduates of an accredited nursing program, hold a current Ohio RN license in good standing, and have at least two years of RN experience, including hospice experience. Strong clinical judgment, communication, organizational skills, and a commitment to patient-centered end-of-life care are essential. A BSN and Certified Hospice and Palliative Nurse (CHPN) certification are preferred. Candidates must have a valid driver’s license, reliable insured transportation, satisfactory references, and the ability to meet required background screening requirements.
