SUMMARY

 

The Journey Care Coordinator is responsible for scheduling and implementing all resident activities and excursions for Journey Care residents with input and assistance from the Journey Care Director. The Journey Coordinator is responsible for all programming, organizing, scheduling, budgeting, facilitating, and directing the overall operations of the activities and excursions. Excursions should be planned weekly with the intention of exploration, creativity, enjoyment, and scenic drives should only be planned once a month unless the weather is unfavorable. Scenic Drives should have a purpose complete with a planned destination, scavenger hunt or “Road Hunt” rather than just driving down road. All responsibilities must be completed in accordance with current standards, guidelines and regulations, company policies and procedures, and as may be directed by the Executive Director to assure that an on-going program of activities, excursions and events are designed to meet the interests, desires and needs of the residents. Assist in hiring, monitoring performance, and supervising assistants in accordance with company policy. Communicate with the Journey Care Director about staff evaluations, merit pay increases, reprimands, disciplinary actions, and terminations. Reports to: Journey Care Director and Programs Director.

 

ESSENTIAL FUNCTIONS, DUTIES AND RESPONSIBILITIES

1. The primary responsibility is to provide an active and engaging atmosphere throughout the community through implementation of calendars created. Promote resident participation through regularly scheduled high-quality activities that offer a variety that stimulates the mind, motivates the body, and rejuvenates the spirit. This includes excursions, minimum of one special event planned, evening movie showings, exercise sessions in morning and afternoon, games, projects with purpose that connect residents to those outside of the community, and other such activities monthly suitable for the level of cognitive skills.

2. Ensure activities and events acknowledge, meet, and celebrate the cultural, religious, and ethnic diversity and interests of the residents.

3. At WellQuest Living Communities, personally meet with each new resident within seven (3) days of move-in to introduce yourself along with asking the likes and dislikes of resident for entry into LifeLoop.

4. Ensure that the department is staffed, and activities are planned seven days a week, excursions a minimum of once a week. There should be no less than four (4) excursions in a month unless a driver is unavailable, or the vehicle is incapacitated.

5. This position may require you to work weekends or a portion thereof including flexibility for an event or activity in the evening.

6. All excursions involve research; making reservations, consent from family for attendance and payment, establishing pricing for excursions, previewing venues, anticipating and meeting comfort needs of residents during excursions, ensuring the safety of residents, communicating with scheduled drivers, and supplying any details needed.

7. Create, develop, promote to family, plan and attend special events held at or hosted by the community relevant to activities or as directed by Journey Director.

8. Actively encourage resident participation in all excursions, events and activities through conversation and direction.

9. Assist in hiring, monitoring performance, and supervision of assistants in accordance with company policy.

10. As directed, plan, schedule, prepare and conduct staff in-services for activity personnel alongside Journey Care Director.

11. Monitor compliance with Department’s budget through the effective and timely completion of Monthly Spend Downs.

12. Ensure compliance with company policy and procedures for proper use of Petty Cash in conjunction with performance of job duties.

13. Ensure all necessary vendor/entertainer invoices are completed accurately, timely and in accordance with all company policies and procedures. This includes the timely and accurate completion of W-9 submissions from the vendor/entertainer when new to community and outside of the scheduled entertainment provided by Programs Director.

14. Ensure knowledge remains current regarding senior activities through completion of offered continuing education courses and networking opportunities with other community professionals.

15. Develop, implement, and manage a “Resident Birthday” program at the community that minimally includes a monthly celebration, day of acknowledgement with card and treat.

16. Develop, recruit, manage and schedule Community Volunteers. Complete and maintain all required logs and paperwork associated with the Community’s Volunteer Program with the assistance of the Journey Care Director.

17. Accurately and timely perform all clerical work connected with the position (e.g., Monthly Activity Calendar, Connection Total to Corporate, Community TV Displays and daily agendas.)

18. Aid in community marketing effort through interactions with all potential residents, family members and referral sources. Greeting each and providing details of activities, events and outings offered.

19. Collaborate with Programs Director in Assisted Living regarding combined events, outings or activities as well as getting scheduled entertainment and outing schedule monthly.

20. Support a positive and professional image through actions and dress attire.

21. Maintain a safe and secure environment for all staff, residents, and guests, following established safety standards.

22. Perform other duties consistent with position and/or as assigned by Journey Care Director or Corporate Director of Programs.

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

 

MINIMUM QUALIFICATIONS

 

Education & Experience: CALIFORNIA – For communities licensed for sixteen (16) to forty-nine (49) persons: · Must have at least six (6) months' experience in providing planned activities or be enrolled in an appropriate education or training program. For communities licensed for fifty (50) or more persons: · Must have one (1) year of experience in conducting group activities and be knowledgeable in evaluating residents’ needs, supervising other employees and in training volunteers. These requirements may be waived if an exception to this requirement is made by the licensing agency upon the community’s written presentation of a satisfactory alternative plan. Prefer one (1) year of senior living experience.

Mathematical Skills: Ability to perform simple mathematical calculations for preparing pricing and producing reports and receipts.

Reasoning Skills: Understand complex issues and ability to carry out plan of action on own initiative.

Oral/Written

Communication Skills Demonstrates sound communications skills, both verbally and written in dealing with residents, staff, families, visitors, and vendors. Must be able to speak, read and write English.

Equipment Used: Be proficient in basic office equipment and standard electronics and technology to accomplish and maintain a high level of job performance.

Physical Effort: Good physical health verified by a health screening, including a chest x-ray or an intradermal test result not more than 6 months prior or seven (7) days after employment. Be mobile and able to perform physical requirements of the job. Assist in loading and unloading residents into community vehicle/van. Will require physical assistance and pushing of residents during activities.

Personal Characteristics: Displays sincere compassion towards cognitively impaired older adults. Demonstrates genuine concern for the physical and emotional needs of older people and their families.

Working Conditions: Well-lit, safe areas with appropriate ventilation and temperature levels. Will accompany residents on outings outside of the community.

Other:

(As required by Title 22) Employee must pass a criminal record clearance prior to beginning work in the community. Employee must possess current first aid training. Employee must be at least eighteen (18) years of age.
