Position Summary
Wellness & Hope Behavioral Health Services PLLC is seeking an energetic, professional, and relationship-focused Community Outreach & Referral Coordinator to expand our referral network and increase new patient referrals.
This is primarily a community-based, in-person outreach position. The ideal candidate will visit healthcare providers, community organizations, and other potential referral partners to introduce our psychiatric services, distribute referral materials, establish professional relationships, and create sustainable referral partnerships.
We are looking for someone who is comfortable walking into offices, introducing the practice, speaking with office managers and healthcare professionals, and following up consistently to develop strong referral relationships.
Key Responsibilities
- Conduct in-person community outreach
- Visit potential referral partners regularly
- Introduce our behavioral health services
- Develop relationships with referral coordinators
- Meet physicians and office managers
- Distribute referral forms and brochures
- Explain our patient referral process
- Identify new community referral opportunities
- Follow up with prospective referral partners
- Maintain relationships with existing partners
- Attend community and networking events
- Track outreach visits and contacts
- Track referrals generated from outreach
- Provide regular outreach activity reports
- Represent the practice professionally

Contractor Expectations
This is a part-time independent contractor position focused primarily on field outreach.
The contractor will be expected to develop an organized outreach schedule, visit targeted organizations in person, document contacts and follow-up activities, and provide regular reports regarding referral development.
Performance may be evaluated using measures such as:
- Number of in-person visits completed
- Number of new referral relationships established
- Follow-up completion
- Number of organizations actively referring
- Growth in qualified patient referrals
- Conversion of outreach contacts into referral relationships
