Summary/Objective
The Wellness Center for Older Adults is a preventive health care facility offering physical and mental health services to adults over the age of 55 and their caregivers. The Wellness Center is located in the Plano Senior and Recreational Center but provides services and outreach at many sites across Collin County. It is a non-profit agency that received financial support through charitable donations, client fees, public and community funding.
We are seeking a nurse to provide compassionate care, direction and education to a growing older adult population and have a servant's heart in working with others. Responsibilities and Duties
This position is a part time, 10-hour per week position. The nurse coordinates his/her schedule and reports to the Head Nurse of the Wellness Center. The Nurse will be responsible for:
- Educate clients on various health topics with emphasis on preventive health
- Planning and performing health services based on the program’s goals and objectives
- Monitor the health status of individuals with chronic disease and assessing individuals with acute illness
- Provide foot care, hand care, ear care, lab tests, blood pressure, and blood sugar checks
- Refer individuals with medical and social needs to community resources/physicians
- Provide and coordinate health education programs to individuals and groups
- Assess cognitively impaired elderly and refer counseling as needed
- Work collaboratively with Wellness Center social workers and counselors
- Assist in the preparation and reporting for grants
Required Education and Experience
A registered nurse, minimum Bachelor’s Degree and licensed to practice in Texas. A minimum of five years’ experience in clinical nursing practice with two-years’ experience in ambulatory or community health settings. Must be innovative, self-motivating, flexible, team-oriented, enthusiastic and enjoy working with older individuals. Must possess professional liability insurance.
Supervisory Responsibility
This position has no supervisory responsibilities.
Work Environment
This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets and fax machines.

While performing the duties of this job, the employee is required to engage in active communication. The employee frequently is required to stand; walk; and reach with hands and arms. This position requires the ability to occasionally lift office products and supplies, up to 20 pounds.

This is a part-time position, 10 hours per week. Exact work schedule to be coordinated with Head Nurse.
Travel
Travel in and around Collin County may be required for outside preventive health clinics.
