Summary / Objective

The Wellness Center for Older Adults provides counseling, community outreach and social service support through a variety of programs for caregivers and senior adults, age 55 and older within Collin and surrounding counties. This role provides opportunities to help caregivers and senior adults find direction, improved coping, decision making, and problem solving through emotional and educational support in individual and group settings. This position will provide supportive counseling, caregiver consultation, and community education with older adults and family caregivers via telephone, in-person office visits, video conferencing, and community outreach settings. We are seeking a licensed Master’s Level professional who enjoys working with older adults and has a degree in Social Work, Gerontology, Counseling, or another related field. Preference will be given to someone with a working knowledge and field experience of issues related to caregiving, aging, and dementia in educational and support group settings. Expected duties for this role include the following tasks:

  • Initial Assessment and Evaluation of psychosocial needs of client (older adults and caregivers).
  • Provides support and caregiver consultation to older adults and caregivers.
  • Provides community education outreach supports.
  • Maintains record of services provided.
  • Develops and facilitates support groups.
  • Develops and maintains ongoing knowledge of community resources for referrals.
  • Stays abreast of current research and strategies, supports, and disease specific resources.
  • Serves as an aging information resource to community.
  • Develops, coordinates, and maintains goals, objectives, outlines, and materials for programs.
  • Collaborates with director and other personnel to provide services.
  • Maintains the electronic and hard copy file structure for all clients.
  • Works with others as support on grant applications.


Competencies

  • Ability to identify, plan, and coordinate short-and long-term goals for clients.
  • Demonstrate knowledge of computer programs and databases including Microsoft Office (Excel, Word, and Outlook, Internet Explorer (ability to learn additional database programs as determined)
  • Must have working knowledge of Internet and ability to correspond via email.
  • Must have excellent time management and organizational skills.
  • Must have excellent telephone etiquette skills.
  • Demonstrate ability to produce professional correspondence and presentations.
  • Demonstrate ability to meet deadlines for agency needs.
  • Demonstrate ability to work as a team player and work effectively with program and community partners, volunteers, clients, and other agency team members.


Supervisory Responsibility

This position has no supervisory responsibilities.


Work Environment

This job operates in a professional office environment. This role routinely uses standard office equipment such as computers, phones, photocopiers, filing cabinets, and fax machines.

The job operates in the office, on-site, and part-time.

  • Remote/telecommute work is NOT an option.
  • Relocation is NOT provided.


Physical Demands

The physical demands described here represent those that an employee must meet to perform the essential functions of this job successfully. While performing the duties of this job, the employee is required to engage in active communication. The employee frequently is required to stand, walk, and

reach with hands and arms. This position requires the ability to (occasionally) lift office products and supplies up to 20 pounds.


Position Type/Expected Hours of Work

This is a part-time position, 22-26 hours per week. Specific schedule to be determined in coordination with the Executive Director. This position is grant-funded and is based on the availability of funds.


Travel

Travel in and around Collin County is required for support groups, community education, and related activities. You must maintain a current Texas driver's license, reliable transportation, and the necessary automotive insurance.


Required Education and Experience

  • Ability to lift at least 20 pounds.
  • Must pass a pre-employment background check.
  • Must be of high moral, ethical and professional character.
  • Master’s Degree in Social Work or related field with five (5) years’ experience.
  • Must have Social Work or Counseling license in the state of Texas
  • Must be able to work a (relatively) flexible schedule to accommodate agency needs.
  • Ability to engage and effectively communicate with persons from diverse cultures and communities.
  • Must maintain a current Texas drivers’ license, have reliable transportation and current automotive insurance.
  • Skills in organizing work responsibly within an environment with multiple challenges, priorities, and time frames.
