Responsibilities
- Assist in planning, developing, organizing, implementing, and evaluating activity and recreation programs.
- Assist in developing a written plan of care (preliminary and comprehensive) for each resident that identifies the problems/needs of the resident and the goals to be accomplished for each problem/need identified.
- Encourage the resident/family to participate in the development and review of his/her plan of care.
- Follow the care plan in providing daily activities for the resident
- Observe resident attendance, mood, behavior, and degree of involvement in order to track resident progress.
Escort patient and residents from assigned rooms to vehicles and appointments as needed by driving the facility transportation
Requirements:
- High School Diploma or equivalent
- Long term care experienced preferred
- Current SRNA in good standing
- Prior recreation/activities experience preferred
