The Home Health Licensed Practical Nurse (LPN) uses an individualized patient care plan to provide top quality care to all of our patients; keeping them Happy, Healthy and at Home. We are seeking a passionate LPN for our weekday team in Guilford County and surrounding areas.
This position is Full-Time Monday- Friday 8am-5pm. Full benefits including: Medical, Dental, Vision, 401k with matching, and immediate PTO accrual.
Licensed Practical Nurse (LPN) visit 6-7 patients on any given shift, driving to the patients home providing skilled nursing services. This position is primarily responsible for the initial assessment and case development of patients in all stages of life in their homes. Duties include but not limited to, assessing, interpreting, planning, implementing and evaluating patients according to the patient’s age and diagnosis.
Licensed Practical Nurse (LPNs) with strong assessment, skilled nursing and documentation skills are strongly encouraged to apply!

We are 100% EMR and clinicians document in the patients' homes with company-issued laptops. LPNs must be okay with traveling throughout the service area!
