What are the responsibilities and job description for the Registered Nurse Case Manager position at Well Life Family Medicine?
Well Life Medicine is seeking a dedicated registered nurse case manager to join our growing team! This will also include assisting with other varied tasks such as patient triage, refill delegation, coordinating with patients and providers after emergency room or hospital discharge and patient education. Plan for one day per week in clinic (more if you like), the rest work from home!
The ideal candidate will possess strong clinical skills and a commitment to providing high-quality patient care. One year of case management required.
Duties
- Maintain and manage our chronic care patients. You will work with their primary care providers to develop and fulfill treatment plans.
- Work in a dyad with our community health worker (CHW): you can focus on their medical needs and the CHW can address their social needs.
- Work with providers regarding patient care, refill requests, phone triage.
- Collaborate with interdisciplinary teams to ensure coordinated care and optimal patient outcomes.
- Provide health coaching and education to patients and their families regarding health management.
- Maintain accurate medical documentation and bill for services (we will teach you if you don't have billing experience!)
- Participate in case management activities to facilitate effective discharge planning.
- Engage in continuous professional development through training and education opportunities.
Experience
Candidates should have a valid nursing license and one year relevant experience in case management. Familiarity with laboratory procedures, ambulatory surgery centers, health coaching techniques, medical documentation practices, case management systems is highly desirable. Strong communication skills, attention to detail, and the ability to work effectively in fast-paced environments are essential for success in this role.
In this hybrid role, you will be the primary driver of our chronic care initiatives—managing the patient panel, executing billing, and performing face-to-face care coordination. Qualifications
- Valid Registered Nurse (RN) license in the state of Oregon.
- 1 year of relevant case management experience.
- Strong independent workflow skills—because you will be working from home part of the week and owning a clinical program, self-motivation and highly organized communication are essential.
- Familiarity with Athena EHR and outpatient care coordination is a major plus, but if you're a quick learner, we can teach you!
