Resident Care Coordinator Opportunity at Aster Place Assisted Living
CNA or HHA!
The Resident Care Coordinator plays a key role in supporting the residents’ well-being by managing healthcare coordination, advocating for psychosocial needs, while supporting health and wellness initiatives.
Skills Needed:
- Clinical and Human Services knowledge: Understanding psychosocial assessments and interventions.
- Interpersonal Communication: Empathetic engagement with residents and families. Clear, effective communication with interdisciplinary teams. Conflict resolution and mediation skills.
- Supportive Presence: Creating a comforting and engaging atmosphere for our residents.
- Collaboration: Works with and promotes collaboration among departments, professionals, consultants, and organizations including government regulatory agencies.
- Teamwork: The ability to work towards a common goal of excellent care for our residents.
Requirements:
- Bachelor’s degree in social work or in a human services field or Social Service Designee certification required.
- Strong passion for geriatric advocacy and commitment to senior care excellence.
- Previous social services experience preferred.
