RESPONSIBILITIES:
Client Services:
- Responsible for the direct care, safety, hygiene and welfare of each client in the site in accordance with each client’s ISP and with respect to the dignity and rights of each client.
- Administer and supervise the clients in the taking of medications and document accordingly.
- Review new MARS and make all needed corrections for your clients and have in client books by the 1st of the month. Review Physician orders; make any correction to send to pharmacy by the 3rd of the month. File previous months MARS and physicians orders in the client’s medical book.
- Assure all your clients meds are ordered on time and in place when needed.
- Maintain, record and review client’s documentation daily. (HPC’s, T-Logs, Time tracking, Documenting Activities in comment section, MARS, Mileage sheets, staff signing in for services .) Address staff if the documentation is missing or inaccurate. If it continues, bring it to the attention of the Manager.
- Provide transportation to the client’s to activities outside the site.
- Assist with medical appointment as needed. Communicate with transporters all needed information on the client’s health for appointments.
- Responsible for the security, domestic chores and light maintenance tasks at the site.
- Communicate with maintenance about needed repairs. Complete facility Dude request as needed for all repairs.
- Maintain positive, proactive and communicative relationships with all client families/guardians to ensure all parties involved with that client stay informed and involved. Intervene immediately to attend to any issues or potential issues that may arise with a family member/guardian, working directly with Residential.
- Maintain up to date knowledge of all clients’ ISPs—including all medical, psychiatric, behavioral, nutritional, social and physical needs and ensure that staff are adhering to each ISP.
- Document issues pertaining to staff corrective action and work with DRS to address.
- Help create activity schedules for each client to ensure that each client has ample opportunity to get involved in their community. Team Leaders will ensure activities are being done by staff. All clients should have a full and involved life. It is your responsibility to ensure that happens.
- Age 21 or older
- Meet DSP job and skill requirements, attributes and values.
- Ability to lift 50 pounds and safely lift, turn, position and transfer individuals with physical disabilities.
- Must have solid communication skills, both oral and written.
- Must have solid computer skills and understanding of MS Word, Excel and e-mail.
- Must have strong organizational, problem solving and time management skills.
- Must demonstrate integrity and ability to lead others.
- Ability to prepare basic meals (read and follow recipes).
- Must have a belief in and understanding of agency’s culture and mission and ability to promote and communicate the philosophy, mission and values to internal and external stakeholders.
- Must have reliable transportation and a valid (Ohio) driver's license with fewer than three violations in the past three years and no convictions of OVI, vehicular homicide, driving without a suspension, or tickets over 10 mph and auto insurance.
- Must have satisfactory health, background check and drug test.
- Flexible schedule to cover call offs, vacations and sick.
- Must obtain CBTLA within 6 months of hire.
- Ability to obtain med-pass certification within 90 days of hire.
- Must provide proof of unexpired CPR/First Aid certification or complete CPR/ First Aid certification during Welcome House orientation. Staff are not permitted to provide direct care unless they have current CPR/First Aid certification.
