We are seeking an energetic and compassionate Rapid Rehousing Case Manager to join our dedicated team. In this vital role, you will empower individuals and families experiencing homelessness by guiding them through the process of securing and maintaining stable housing. Your proactive approach and expertise will help transform lives, ensuring each person receives personalized support, resources, and advocacy to achieve long-term housing success. Duties
- Assess clients’ needs and develop individualized housing plans aligned with program goals and funding requirements.
- Assist clients in navigating landlord-tenant laws, lease negotiations, and understanding Fair Housing regulations to secure suitable housing options.
- Coordinate with property management teams, landlords, and service providers using platforms like Yardi or OneSite to facilitate placements and track progress.
- Provide crisis intervention and crisis management support to clients facing urgent challenges or setbacks during their housing journey.
- Utilize motivational interviewing techniques to foster client engagement, build trust, and promote self-sufficiency.
- Conduct regular follow-ups with clients post-housing placement to ensure stability, address barriers, and prevent eviction or homelessness recurrence.
- Maintain accurate documentation of case notes, service plans, and progress reports in compliance with program standards. Skills
- Strong knowledge of HUD programs, Section 8 housing vouchers, and landlord-tenant law.
- Proficiency in property management software such as Yardi or OneSite for data entry and reporting.
- Expertise in negotiation, crisis intervention, addiction counseling, and crisis management strategies.
- Familiarity with Fair Housing regulations and Section 8 policies to ensure fair access to housing opportunities.
- Excellent social work skills combined with motivational interviewing techniques to empower clients effectively.
- Ability to handle sensitive situations with professionalism while maintaining a positive outlook.

- High school or equivalent (Required)
Experience:
- social services: 1 year (Preferred)
