The PSH Service Coordinator is a full-time professional role focused on assisting individuals experiencing homelessness by providing support to secure stable housing, income, and necessary services. Key Responsibilities:
- Service Coordination:
- Manage a caseload of up to 30 households in scattered-site and site-based housing programs.
- Conduct housing assessments, eligibility reviews, and action plans for participants.
- Assist with housing searches, landlord engagement, and move-in processes.
- Maintain accurate documentation in the HMIS system (Vesta).
- Participant Engagement:
- Conduct monthly and bi-monthly home visits.
- Provide advocacy, budgeting assistance, and connections to support services.
- Offer transportation when needed.
- Administrative Duties:
- Ensure timely completion of assessments, certifications, and documentation.
- Enter data into databases within three business days.
- Manage rental and utility assistance forms.
- Collaboration and Community Relations:
- Build and maintain relationships with landlords and community organizations.
- Represent the agency at meetings and events.
- Ensure smooth communication with internal and external stakeholders.
- Reporting and Evaluation:
- Maintain compliance with agency guidelines and funding requirements.
- Conduct regular audits of participant files.
- Stay informed about best practices in supportive housing.
Qualifications:
- Education: Bachelor’s degree in Social Work, Human Services, Sociology, or Psychology preferred.
- Experience: Experience in homelessness services, case management, substance abuse, or mental health is a plus.
- Skills: Strong communication, problem-solving, critical thinking, and conflict management skills. Familiarity with HMIS and local resources is advantageous.
- Additional Requirements: Valid driver’s license, car insurance, and flexibility to travel (30%).

- Proficient documentation, data entry, case planning, and engagement.
- Timely completion of assessments and compliance with policies.
- Strong knowledge of mental health/substance abuse resources.
Working Conditions:
- Environment: Combination of office, participant homes, and off-site locations.
