What are the responsibilities and job description for the Representative Payee Case Manager position at Welcome House of Northern Kentucky?
The Protective Payee Service Coordinator is a part-time professional, direct service position responsible for providing case management and advocacy working with program participants who have a mental and/or physical disability or are a victim of crime and are participants in the Representative Payee Program. This position is responsible for engagement, assessment, intervention, and coordination of services to ensure participants’ housing, income, health, and basic needs are met. The Service Coordinator also assists with development and maintenance of the case plan and budget.
- Case Management & Participant Support – Manage a caseload of ~25 participants, develop case plans, conduct regular check-ins, and provide advocacy to ensure stability.
- Financial & Benefits Coordination – Assist with budgeting, Social Security benefits, Stable Accounts, and financial management to ensure participants' needs are met.
- Housing Assistance – Help secure and maintain stable housing, collaborate with landlords, and assist with relocations if needed.
- Community & Agency Collaboration – Build relationships with service providers, government agencies, and community partners to connect participants with essential resources.
- Program Administration & Compliance – Maintain accurate records in HMIS (Vesta), submit required reports, and ensure compliance with program policies.
- Transportation Support – Arrange or provide transportation for participants to essential appointments, including housing, medical, and financial services.
- Public Engagement & Advocacy – Represent the agency in external meetings, promote program awareness, and support participant self-advocacy.
Effective communication with clients, their families, and other stakeholders is essential.
Education:
Associate degree to Bachelor’s Degree or equivalent desired
Experience:
A minimum of 1-3 years’ experience in the social service field
