What are the responsibilities and job description for the Licensed Practical Nurse (LPN) position at Welcome House, Inc?
Responsibilities:

Assist with and support annual self-medication administration assessments under RN supervision
· Support the coordination and documentation of Individual Specific Trainings for assigned clients
· Monitor and document clients’ health conditions and report significant changes to the RN and/or physician
· Provide input and assistance in implementing physician’s orders and health care plans
· Attend medical appointments with clients when authorized by the Director of Nursing
· Provide instruction to direct care staff on routine health-related matters, hygiene, and medication protocols, under RN guidance
· Administer medications and treatments as authorized by the physician and under the delegation/supervision of an RN
· Participate in staff training related to medication administration, health, and safety
· Accurately document health concerns, medication administration, and treatment in CaraSolva and other systems
· Review and check shift communication (Brittco, CaraSolva, shift book) for updates and report relevant information
· Communicate effectively with the care team, including Support Administrators and QIDPs, as needed
· Assist in preparing and proofreading monthly medication administration records (MARs)
· Ensure documentation of verbal orders, medication changes, and new prescriptions are properly relayed and implemented under the direction of the RN
· Track completion of labs, consults, and follow-up medical appointments
· Support the organization and storage of medications in compliance with regulatory requirements
· Notify the supervising RN or DON of medication errors, unusual client health changes, or staff concerns
· Participate in annual medication administration skills checks and documentation as directed
· Assist with training and supervision of direct care staff delegated to administer medications
· Maintain respectful and professional relationships with clients, families/guardians, and healthcare providers
· Participate in nursing team meetings and trainings as assigned
· Be available to respond to calls for assigned clients outside of working hours, if designated
· Support care coordination during client hospitalizations and transitions back to the group home
· Complete post-discharge assessments under the direction of the RN within 24 hours, when required
· Maintain confidentiality and comply with HIPAA and agency policies
· Perform other duties as assigned by the Director of Nursing or Chief Executive Officer
