Welcome Home Health LLC is currently seeking a compassionate and dependable Registered Nurse (RN) to join our growing home health team. We are a Medicare & Medicaid-certified agency proudly serving patients throughout Palm Beach, Martin, St. Lucie, and surrounding counties.

Boca Raton Delray Beach Boynton Beach Lake Worth West Palm Beach Wellington Royal Palm Beach Palm Beach Gardens Jupiter Stuart Port St. Lucie Fort Pierce and surrounding areas.
Position Summary
The Registered Nurse (RN) is responsible for providing skilled nursing services to patients in their homes in accordance with physician orders and the plan of care. The RN will complete assessments, coordinate care, educate patients and families, and ensure quality patient outcomes.
Responsibilities
- Perform Start of Care (SOC), Resumption of Care (ROC), Recertification, and discharge visits
- Complete comprehensive nursing assessments and documentation
- Develop and update individualized care plans
- Administer medications, wound care, IV therapy, and other skilled nursing services
- Educate patients and caregivers regarding disease management and safety
- Coordinate care with physicians, therapists, case managers, and interdisciplinary team members
- Ensure compliance with Medicare, Medicaid, and agency guidelines
- Complete all documentation accurately and timely in EMR system
Qualifications
- Active Florida RN License
- Minimum 1 year of nursing experience preferred
- Home health experience preferred
- Valid driver’s license and reliable transportation
- Current CPR Certification
- Strong communication and organizational skills
- Experience with EMR documentation preferred
- Bilingual (English/Creole or English/Spanish) is a plus
