The Behavioral Health Clinician (BHC) is a core member of the Primary Care Behavioral Health Team at Family Medical Center and Weiser Memorial Hospital. The BHC is responsible for identifying individual and family psychosocial, emotional, and cultural needs and priorities for care through assessment, care planning, clinical interventions, education, and evaluation of outcomes. The BHC is responsible for supporting and coordinating the mental and physical health care of clients with the patient’s medical provider and, when appropriate, other members of the health care team in both the clinic and the Emergency Department. The BHC provides counseling and support to alleviate stress, enhance functional coping abilities, minimize potential problems/barriers related to treatment and maximize opportunities for communication and problem-solving.

Weiser, ID, is located in Western Idaho, minutes away from I-84 and offers a lower cost of living than the national average, including nearby Boise. Responsibilities: 

 

  • Complete psychosocial risk assessments, intake/diagnostic evaluations, and crisis intervention with clients, including the family and other social support systems as necessary. 
  • Support the mental and physical health care of clients with the patient’s medical provider and, when appropriate, other members of the health care team. 
  • Determine priorities for clinical interventions with special consideration given to complex and/or high-risk issues. 
  • Determine differential treatment needs of clients and families based upon assessment data, psychological theory and standards of clinical practice. 
  • Screen and provide specialized focused assessments (i.e., suicide risk, mental health issues, chemical dependency, and abuse). Facilitate patient engagement and follow-up care. 
  • Provide client education about common mental health and substance abuse disorders and available treatment options. 
  • Systematically track treatment response and monitor patients (in person or by telephone) for changes in clinical symptoms and treatment side effects or complications. 
  • Support psychotropic medication management as prescribed by medical providers, focusing on treatment adherence monitoring, side effects, and effectiveness of treatment. 
  • Document assessment, plan of care, interventions and measurable goals and outcomes on interdisciplinary record in compliance with third party billing requirements. 
  • Provide brief behavioral interventions using evidence-based techniques such as behavioral activation, problem-solving treatment, motivational interviewing, or other treatment as appropriate. 
  • Facilitate support groups as necessary and appropriate. 
  • Provide crisis intervention services to patients, families, and staff, both in the clinic and Emergency Department. 
  • Provide social work consultation, (i.e., maintain updated and ongoing awareness of social service agencies, programs and entitlements relevant to clients and families). 

Requirements

 

  • Master’s degree in social work, or Master of Arts degree in counseling, or master’s degree in marriage and family therapy, or similar degree from an appropriately recognized institution of higher learning.
  • Maintain Current LCPC, LPC, LMFT, LMSW, LCSW or higher licensure in Idaho.
  • Two-years’ experience in a clinical practice providing mental health assessments, care planning and counseling.
  • On-call as assigned (1 week on, flexibility in schedule and telehealth option)
  • Accurate documentation that complies with standards and requirements for third party billing.
  • Excellent communication skills to include oral and written comprehension and expression.
  • Ability and willingness to work with clients’ growth and development needs, particularly related to the age of clients in the unit/area(s) of assignment. Use of critical thinking skills, social work theory practice.
  • Ability to manage a chaotic work environment related to changing client needs, including work with clients with acute, chronic, and complex disease processes, and those who are dying.
  • Ability and willingness to exhibit behaviors consistent with standards for performance improvement and organizational values.
  • Ability and willingness to exhibit behaviors consistent with principles for service excellence.
