Responsibilities
- Provide community-based support services to individuals with mental health needs.
- Assist clients in developing skills related to daily living, coping, medication management, and community integration.
- Complete progress notes and documentation in accordance with program and state requirements.
- Coordinate with therapists, families, medical providers, and other agencies to support client goals.
- Monitor client progress and adjust support strategies as needed.
- Participate in treatment team meetings and ongoing professional development.
- Maintain confidentiality and adhere to all ethical, legal, and agency standards.
- Qualifications
- Bachelor's degree in psychology, social work, or related field.
- Strong interpersonal and communication skills.
