This is a Full-Time Hybrid position. This flexible position offers autonomy while utilizing your nursing assessment, care planning, nursing oversight and case management skills. The WeCare 365 Adult Foster Care program is licensed by the State of Massachusetts to provide much needed support to our members and those that are caring for them. The AFC program provides clinical support and a stipend to caregivers for the valuable work they do. We are committed to caring 365 days a year with care plans carefully tailored for member needs through a team approach for holistic, wrap-around services. We change lives one family at a time by recognizing their diversity and support them with professional staff who are multilingual and multicultural. WeCare 365 provides caregivers and member families with financial relief and the comfort that comes with comprehensive service.

EXPERIENCE:
- Registered Nurse or Licensed Practical Nurse in Massachusetts
- Experience with Minimum Data Set (MDS) assessment tool a plus
- Care Management experience.
- Monthly, Bi-Monthly visits (in person and/or telephonic)
- Strong communication skills
- Medication Management
- Medical documentation and transcription
- Experience with databases and Microsoft Office
- Caregiver Training and Education
- Must be able to travel and have a reliable vehicle.
