Job Summary
We are seeking a per diem Registered Nurse (RN) with knowledge and experience working with individuals served through the New Jersey Division of Developmental Disabilities (NJ DDD). Experience with DDD medical acuity requirements, nursing assessments, staff training, care coordination, and compliance with DDD policies is strongly preferred. The RN will provide nursing oversight and consultation as needed to support individuals receiving services. An active New Jersey RN license and reliable transportation are required.
Responsibilities
- Conduct comprehensive initial nursing assessments for individuals receiving services.
- Develop, implement, and update individualized nursing care plans based on assessed needs.
- Complete required nursing documentation in accordance with New Jersey DDD, Medicaid, and agency standards.
- Provide monthly in-person nursing visits to assess health status, review care plans, and identify changes in medical needs.
- Conduct weekly or biweekly telephone follow-up calls (as clinically indicated) with individuals, families, DSPs, or other caregivers to monitor health concerns, medication issues, and overall well-being.
- Collaborate with physicians, support coordinators, guardians, and interdisciplinary team members to ensure continuity of care.
- Train and educate Direct Support Professionals (DSPs) on individualized health care plans, medication administration (when applicable), infection control, and other health-related needs.
- Monitor individuals with medical acuity needs and recommend interventions when appropriate.
- Identify changes in condition and coordinate timely medical evaluation or emergency services when necessary.
- Participate in interdisciplinary team meetings and provide nursing recommendations.
- Ensure compliance with all applicable NJ DDD, Medicaid, HIPAA, and agency policies and procedures.
- Maintain an active New Jersey RN license and required continuing education.
- Perform comprehensive patient assessments, including vital signs, physical examinations, and detailed medical histories.
- Ensure compliance with HIPAA regulations and maintain accurate medical documentation while adhering to NCQA standards and ICD coding practices (ICD-9/10).
Qualifications
- Current New Jersey Registered Nurse (RN) license in good standing.
- Experience working with individuals with intellectual and developmental disabilities (I/DD), preferably within the NJ DDD system.
- Knowledge of NJ DDD regulations, medical acuity requirements, individual and community-based supports.
- Excellent assessment, communication, and documentation skills.
- Reliable transportation and willingness to travel to individuals’ homes.
